| Attendance blanket permission form | This form only needs to be completed once to state that your students will be learning from home. |
| On-site attendance form | This form to request on-site learning supervision must be completed by 3:30pm on the Thursday prior to the week in which supervision is required. The principal will then make contact with the parent/carer on the Friday to approve or deny this request. |
